1. Use only one set of forms for 2 vehicles concerned (2 sets of forms for 3 vehicles concerned, etc.). It is unimportant who provides and fills in the form. Use the ballpen and fill in the form properly so that even copies were readable.    

2. You shall pay your attention to the following notice when filling in the “Report of a Traffic Accident“:

·         Questions in point 8 relate to your insurance certificate (number of your liability insurance policy, green card number),

·         Questions in point 9 relate to your driving licence,

·         Mark exactly the collision point (point 10),

·         Mark by crossing the option (No. 1-17) which relates to your traffic accident (point 12) and at the end write down the number of options marked,

·         Draw the traffic accident sketch (point 13).

3. Do not forget to mention possible traffic accident witnesses, their names and addresses, especially if your opinion differs from the one of other traffic accident participants.

4. Sign the “Report of a Traffic Accident“ and let it sign to other driver. One copy hand over to the other participant and the second copy hand over to your insurer.

After you fill in the form

·         When advising of the traffic accident to the insurance company do not forget to mention where and when your vehicle could be inspected by an expert,

The form hand over immediately to the respective insurance company.
