1)
Find
out from the offender if possible by examination of his/her documents:
a. his/her
name, surname and place of residence,
b. name,
surname and place of residence or trade name and place of business of
the owner of the
vehicle,
by which the damage was caused,
c. registration
(plate) number and international code of the country of
registration of the vehicle, by which the damage was caused,
d. trade
name and place of business of the insurer, with which the Motor
Third-Party
Liability
insurance was concluded, number of the insurance policy,
e. if
the vehicle, by which the damage was caused, is registrated abroad, find
out also number of the
Green
Card and its time validity (complete data from its column No. 3,4)
or
number of the frontier insurance of such
vehicle
and if it is possible make a copy of these documents.
2) With
other parties involved in the accident fill in the form of the loss
event (it is suitable to have such a form handy), the design of
which may differ by individual insurers, or eventually use the form
„European Report of a Traffic Accident“. If you do not have any such
form available, write down if possible together with other participants
involved in the accident a written document, in which you state all
basic identification data regarding the accident (date, time and
place of the accident), the parties involved and the involved
vehicles and describe, how the accident occurred and what damage and to
whom was suffered. Do not forget the witnesses, plan or sketch of the
place of the accident, photo documentation of the place of the accident
etc.
3)
In
case that:
a. a
person is
injured or killed upon the traffic accident,
b. a
material
damage occurs on any of the involved vehicles including the transported
items or other property exceeding the amount of CZK 100.000,-,
c. a
material
damage was caused to a property of a third party,
report
the traffic accident without any delay to the Police of the Czech
Republic.
If the
accident is investigated by the Police of the Czech Republic, sign the
police record only if you agree to its wording. Otherwise include your
reservations in it.
4) If
no injury or death occurred upon the accident and the originating damage
has not exceed CZK 100.000,- you do not have to report the accident to
the Police of the Czech Republic, if you agree with other participants
involved in the accident on the fault. The form of the loss event may be
considered to represent such a written agreement, if it results from its
content explicitly who caused the damage and if all persons, to whom the
agreement on fault in the accident concerns, also sign this form.
5) Write
down the names and addresses of the drivers of other damaged vehicles,
and data on these vehicles (vehicle registration (plate) number,
production make and type and colour of the vehicle) and the
witnesses of the traffic accident – if possible as much in the scope of
the data as specified in clause 1).
6) Advise
the possible interested person among the participants of the traffic
accident your name and address.
7) With
the presentation of your claim for compensation and with all relevant
documentation, which could prove the liability of the driver or operator
of other vehicle for this damage, you should apply to the relevant Motor
Third-Party Liability insurer of the offender, or to the
Czech Insurers‘ Bureau
, in case
the claim for compensation from guarantee fund
is concerned. There please specify, prove and support by documentation
of sustained damage.
In
case you could not find out from the offender the information about
his/her Motor Third-Party Liability insurer, you can search this insurer
in the database of the Czech Insurers‘ Bureau either via
Internet , or via phone number

with Czech
Insurers‘ Bureau please always
state information about the traffic accident and about the offender’s
vehicle as much as possible. |
